General Declarations
  1. I apply to become a member of the Retirement Protect 90 Pension Plan, a registered pension scheme operated by Quai Investment Services Limited, and agree to abide by the Rules of the Scheme, which may be amended from time to time.
  2. I acknowledge that I have had the opportunity to carefully read the following documents that form the contract I am agreeing to through this application:
   - Retirement Protect 90 Pension Plan Key Features Document
   - Retirement Protect 90 Pension Plan Terms & Conditions
   - Retirement Protect 90 Pension Plan Investment Terms
   - Retirement Protect 90 Pension Plan Brochure Fund Factsheet
  3. I confirm that I am not a US Person as defined by the US Internal Revenue Service (available on www.irs.gov).
  4. I authorize Quai Trustees Limited and Quai Investment Services Limited to open necessary accounts to action my investment instructions and agree to their terms and charges.
  5. I confirm that all information provided by me in this application is correct and complete to the best of my knowledge.
  6. I accept full liability for any claims, losses, or expenses incurred by Quai Trustees Limited due to incorrect information provided by me.
  7. I agree to inform Hilbert Investment Solutions within 30 days of any changes in the details I have provided.
  8. I will not attempt to withdraw funds held for me under the Scheme except in accordance with the Scheme's Rules.
  9. In case of unauthorized payment, I agree to Quai Investment Services Limited deducting any scheme sanction or charge levied by HMRC from my funds to pay the charge to HMRC.
  10. I acknowledge that under the Retirement Protect 90 Pension Plan Terms & Conditions, I am only entitled to receive money as an 'Authorized Payment'.
  11. I acknowledge that death benefits are payable to my beneficiaries at the discretion of Quai Investment Services Limited, considering my Expression of Wishes but not bound by it.
Information Declarations
  1. I acknowledge that the operation of my pension plan relies on Hilbert Investment Solutions Infinity platform communications.
  2. I will maintain and monitor my provided email address and inform Hilbert Investment Solutions immediately of any changes.
  3. I confirm that the address provided in the application is my permanent residential address and will notify Hilbert Investment Solutions within 30 days of any changes, including name changes.
  4. I confirm I have not flexibly accessed any benefits from other pension arrangements and will inform Hilbert Investment Solutions within 90 days if I do.
  5. I understand it is my responsibility to provide evidence of any Lifetime Allowance protections to avoid charges and to inform Hilbert Investment Solutions if I lose or have revoked any protections.
  6. I give my permission for my personal data to be processed per the Privacy Statement and UK GDPR.
  7. I agree to checks to establish proof of my identity and residence and those of a third party or employer where contributions are paid by them.
Financial Advice Declarations
  1. I acknowledge and accept that Hilbert Investment Services Limited and Quai Investment Services Limited do not provide financial, investment, tax, legal, or any other form of advice under the Financial Services and Markets Act 2000.
Risk Declarations
  1. I understand that the value of investments can go down as well as up, and I may get back less than invested. My capital is at risk.
  2. I understand that instant access cannot be guaranteed, and this should be considered a long-term investment.
  3. I understand that tax rules can change in the future.
Transfer-In Declarations
  1. I confirm my wish to transfer pension benefits from my transferring pension scheme(s) detailed in my application into the Retirement Protect 90 Pension Plan and instruct relevant parties to arrange those transfers.
  2. I authorize and apply to the current provider to transfer sums and assets directly to Quai Investment Services Limited and provide any required instructions.
  3. I accept responsibility for any claims, losses, expenses, additional tax charges, or penalties incurred by Quai Investment Services Limited, Hilbert Investment Solutions, and the current provider due to incorrect information provided by me.
  4. I authorize Hilbert Investment Solutions, Quai Investment Services Limited, the current provider, any contributing Employer, and any financial intermediary named in this application to obtain and release information to enable the transfer of sums and assets.
  5. I confirm the value of all pensions and tax-free cash taken combined with benefits now are below the current standard Lifetime Allowance.
  6. I accept responsibility for any additional tax charges or penalties due to incorrect information provided by me.
  7. I confirm that I have not sought or been given any pension transfer advice from Quai Investment Services Limited or Hilbert Investment Solutions.
  8. I understand the potential impact on HMRC protection if transferring to the Retirement Protect 90 Pension Plan.
Transfer-In Declarations
  1. I understand that contributions not eligible for UK pension relief cannot be paid into this pension plan. If contributions eligible for tax relief are paid, I declare that:
   - I am under age 75 and a relevant UK individual.
   - The total contributions to this scheme and other registered pension schemes will not exceed the higher of the basic amount or my relevant UK earnings in any tax year.
   - This declaration is correct to the best of my knowledge and belief. - I will notify Hilbert Investment Solutions if I cease to be entitled to relief on contributions.
  2. I acknowledge that if I have Enhanced or Fixed Lifetime Allowance protection, it will be lost if contributions are made to this pension plan.
  3. I understand that contributions received cannot generally be refunded unless Excess Contribution Lump Sum Payments rules apply or I cancel my pension plan within the 30-day cancellation period.
